**Your registration is complete only when we have received full payment for your class.**

In the upper right corner of our website you will see the Parent Portal button. 

Click to set up your account and download our App.

Please also fill out our Student Information Form.

If you have more than one child you need to fill out a form for each child. You can go to the button below to view and download it. We can also email or fax it to you, or you can stop by our office to get one.​

Here's a step by step on how to fill it out:

1. Fill out personal information

2. Class Information:

You can get complete class information by choosing "Programs" in the navigation bar at the top of the page.

Once you have determined what program you want your child enrolled in, place that information within the form.

We will inform you if we have to use your second choice of classes.

3. Payment Information:

We currently list tuition for the full session only.

You can find this information within the program you have selected for your child.

After the first week of the session, call our office at 616-866-8731 or email us at northstargym@northstargym.net for the prorated amount.

4. Discount Amount:

Multi-Child Discount: This is for immediate family (brothers and sisters only). 

If you have a second child taking classes, you get 5% off your tuition.

If you have three or more children in the program, you get 10% off your total class tuition.

5. "Acknowledgment of Risk & Waiver of Liability" and "Permission to Treat":

These need to be filled out, signed, and dated.

We CANNOT except any student without this form.

You CANNOT alter the form in any way.​

6. There are 4 ways to complete your registration:

a) Bring it to Our Office: 

You can use any method of payment, EXCEPT American Express

b) Online: 

At the top of this page are the directions to register online.

c) Mail:

Please do not mail cash. If you are using a credit card, please fill out the information on the form.

d) Email us at northstargym@northstargym.net: 

Send your form as an attachment such as PDF or JPEG. The ONLY form of payment via e-mail is credit card.​​
