During the gymnastics calendar year, we take photographs/videos of activities involving athletes to share the gym's positive vibe and updates. Some photographs may capture your child's participation, directly or indirectly, These photos/videos may be published through website & social media pages (Facebook, Instagram, TikTok). With this, we seek your consent in allowing us to publish photos that may involve your child. Please provide your response by selecting your choice below and submitting this form.

With my signature below I grant permission for my child(ren) to be photographed, or their images recorded for print or electronic use in promoting the gym's services.

I understand that it is my responsibility to inform NorthStar Gymnastics Inc. in the event that I no longer wish to authorize the above uses. I agree that this form will remain in effect during the term of my child's enrollment within the NorthStar Gymnastics Inc. I understand that there will be no payment for me or my child's participation in this release.
